Maternal/Child Home Visiting Program for Rural America
A model for supporting newborns and their families has emerged as a successful evidence-based program that is being implemented in rural sites around the country. The mission of the program is to mitigate maternal/infant mortality risk by expanding the reach of support in rural communities through effective extension of the newborn’s medical home. The Family Connects model offers an in-home health and social determinants assessment of both the caregiver and newborn by a nurse at three weeks postpartum. The nurse may also make up to two more home visits. This is a strong evidence-based home visiting program that is an easy way for funders to support rural communities.
